Review of intraoperative TOE

چکیده

Introduction In 2000, Westmead Hospital Department of Anaesthesia made a survey of the use of intraoperative transoesophageal echocardiography (TOE) by cardiac surgical units throughout Australia. At that time, 80% of cardiac units were using intraoperative TOE, of which the anaesthetist was primarily responsible for the interpretation in 72%. Were we to conduct the same survey in 2009, I believe we would be hard pressed to find a unit where intraoperative TOE is not an essential component of practice for the cardiac anaesthetist. For valvular surgery, and for valve repairs in particular, intraoperative TOE has become a standard of care. While there is still some debate about the necessity and cost benefit analysis for its use for the routine coronary artery bypass graft (CABG) patient with good left ventricular function, it is also true that with advances in interventional cardiology, these less complicated patients are likely to be undergoing stenting procedures instead of surgery. The older, sicker, patient with multiple co-morbidities who may be undergoing combined valve coronary artery grafting surgery or re-do surgery now makes up an increasing proportion of our workload. The purpose of this review is to outline the current status and role of intraoperative TOE in the cardiac surgery unit as well as the particular Review of intraoperative TOE

The impact of intraoperative transoesophageal echocardiography on decision-making during cardiac surgery.

BACKGROUND Real time intraoperative transoesophageal echocardiograpgy (TOE) has an expanding role in peri-operative management and surgical decision making. OBJECTIVES Studies of the effect of transoesophageal echocardiography (TOE) on intraoperative decision making commonly emphasise major changes in operative plans. We examined more subtle effects using a novel scale, recording influences o...

Use of transoesophageal echocardiography during cardiac arrest in patients undergoing elective non-cardiac surgery.

BACKGROUND Sudden unexpected intraoperative cardiac arrests in patients undergoing elective non-cardiac operations are rare but catastrophic complications. The efficacy and utility of transoesophageal echocardiography (TOE) in the diagnosis and management of these events have not been reported earlier. METHODS Unexpected intraoperative cardiac arrests in patients undergoing elective non-cardi...

The Role of Transesophageal Echocardiography in the Intraoperative Period

The goal of hemodynamic monitoring and management during major surgery is to guarantee adequate organ perfusion, a major prerequisite for adequate tissue oxygenation and thus, end-organ function. Further, hemodynamic monitoring should serve to prevent, detect, and to effectively guide treatment of potentially life-threatening hemodynamic events, such as severe hypovolemia due to hemorrhage, or ...

Transoesophageal echocardiography in cardiac and vascular surgery: implications and observer variability.

Transoesophageal echocardiography (TOE) has gained widespread acceptance among cardiac anaesthetists as a tool to facilitate peri-operative decision-making. This observational study analyses the impact of TOE and its inter-observer variability on intra-operative patient management during cardiac and major vascular surgery. From June 1996 to December 1998, standardized reports were obtained from...
